Standard EEET forms

All the EEET forms available below are "fillable" forms. The "download it" links below are all set to open the form on your computer. If you do not immediately see the form in the pop-up window (and see a "Please wait ..." message), then your internet browser is not set up to view PDFs online; please follow instructions below.

  • NOTE: you do NOT need to login to OneDrive to access these EEET forms. When you click on one of the links below, it is likely you will see something like the image below. IGNORE THE "PLEASE WAIT ... " MESSAGE. You need to DOWNLOAD the form. Clicking on the download links OR the images below will save a copy of the EEET form on your computer. Navigate to your "Downloads" folder on your computer (or wherever downloads are saved on your computer) and open the downloaded form in either ADOBE ACROBAT OR ADOBE READER (Reader is the free version of Adobe). Note that your machine's settings may be configured to automatically open the PDF in your internet browser. If this is occurring, close the internet window that appears, manually open Adobe Reader/Acrobat from your list of programs, then open the file from its downloaded location.

EEET forms

EEET Cover Sheet Image of EEET Cover Sheet

What is it?

A form used to collect information about your event; keep us updated with your most current information, title,
program area affiliation and more.

When should it be used?

With ALL paper EEET submissions.  Every use of a EEET, including Form I (versions 1 and 2), II, III, and Multiple Instructor (a separate cover sheet should be submitted for each instructor) style forms should be accompanied by a cover sheet.

Group - Form I (Version 1)

What is it?

A form used to assess quality of TEACHING; contains 9 variables; in booklet-style format; fillable form (event title, instructor name, and event data may be filled out online and printed -- copies for class made from online print-out). A sample image of the form appears to the right -- note the 9 variables, laid out in a matrix-style.

When should it be used?

At the end of a meeting / teaching session/teaching event. This form is intended to be used with groups of five (5) or more individuals.

What is it?

A form used to assess quality of TEACHING; contains 9 variables; front-back / full-sheet format; fillable form (event title, instructor name, and event data may be filled out online and printed -- copies for class made from online print-out). A sample image of the form appears to the right -- note the 9 variables, each laid out as a separate question with its own rating scale.

When should it be used?

At the end of a meeting teaching session/teaching event. This form is intended to be used with groups of five or more individuals.

Group - Form II EEET Group Form II Image

What is it?

A form used to assess quality of TEACHING and MATERIALS; contains 9 variables (identical to Form I and I.2) PLUS 7 variables regarding materials/content**; booklet-style format; fillable form (event title, instructor name, and event data may be filled out online and printed -- copies for class made from online print-out). A sample image of the form appears above -- note that the 9 variables from forms I and I.2 appear on the right-hand side of the form, and 7 variables concerning the materials and content appear on the left-hand side of the form.

Group - Form III EEET Group Form III Image - Low Literacy

What is it?

A form used to assess quality of TEACHING for low-literacy audiences; contains 9 variables** (variables are slightly different from Forms I and I.2); booklet-style format; fillable form (event title, instructor name, and event data may be filled out online and printed -- copies for class made from online print-out). A sample image of the form appears to the right -- note that there are 9 variables, but they differ slightly from the 9 variables that appear on Forms I, I.2, II, and Multiple Instructor.

When should it be used?

At the end of a meeting/teaching session/teaching event. This form is intended to be used with groups of five or more individuals -- either those with low-literacy skills or children.

Group - Multiple Instructors EEET Multiple Instructors Form Image

What is it?

A form used to assess quality of TEACHING; contains 9 variables; front/back full-sheet style format; designed to record teaching feedback for up to four instructors on a single sheet; fillable form (event title, instructor name, and event data may be filled out online and printed -- copies for class made from online print-out)

**On the FILLABLE form type, when you type in the instructor names on the front page (p. 1), the form will auto-fill the column headers on the back (p.2) with the corresponding instructor names. If you DO NOT TYPE in your information using the fillable format feature, you MUST make sure instructor names match on the front and column headers on the back BEFORE copying the forms (please PRINT legibly). If they do NOT match (or names are missing on page 2), we can NOT process the data.
