Instructions for Using Paper EEET Group Forms (I, II, III & Multiple Instructors):
- EEET instruments are intended for face-to-face teaching with five (5) or more participants.
- Provide separate EEET forms for each instructor OR use copies of the Multiple Instructors Form.
- If using the Multiple Instructor form, designate an educator for the columns (A,B,C,D) and put their names on the EEET form before copying and distributing.
- Complete a separate EEET cover sheet for each instructor and set of EEET forms.
- Do not delete, change, or alter the wording or the order of any item on the instruments.
- Prepare an envelope for the completed forms to be placed in.
- Identify someone (volunteer) other than the instructor/educator (a participant, co-worker, or other person) to distribute the forms to the participants.
- Read the instructions with a sample question to participants. Encourage participants to respond to each question. Encourage participants to write comments and suggestions in the space provided*. Ask the participants not to identify themselves on the forms.
- Then leave the room and allow the volunteer to proceed to distribute and collect forms.
- Give participants at least 10 minutes to complete the instrument.
- Have the volunteer collect, place in envelope, seal, sign their name across the seal and return to the instructor.
- ***NEW as of 10/04/2024*** Submitting completed paper EEET forms. Have an office support staff person (or someone other than the instructor) scan the completed EEET forms and upload them to the embedded Wrike form below. Before scanning the EEET forms, please make sure to:
- arrange all forms so they are facing the same direction;
- remove any forms that are completely blank from the stack;
- include the cover sheet(s) as the first page(s) of the scanned file;
- use two-sided scanning; and
- name the file: "InstructorALastName-InstructorAFirstInitial_EventTitle_yyyy-mm-dd" (event date) e.g. "BuckeyeB_Be a good Buckeye_2025-01-31"
*Note: No summary is made of written comments on paper EEETs, nor is any data entered into the database other than the nine variables. However, all forms are confidentially returned to the Extension instructor and a statistical summary of the nine variables will be available on the EEET Dashboard in Tableau (go.osu.edu/EEET_Dashboards). Please keep in mind that paper EEETs are processed by hand. To check on the status of your paper EEET scans, visit go.osu.edu/PaperEEET_statusReport. Use "CTRL + f" to search for "Instructor A's" last name. When the status is "uploaded to dashboard" you will find the summary on the EEET Dashboard at the link above.
